It does not speak only about Phe as a laboratory result. It tries to connect several dimensions of PKU: elevated phenylalanine levels, neurocognitive symptoms, comorbidities, chronic restriction of natural protein, and quality of life. The document comes from PTC Therapeutics, so it should be read with the necessary caution when a pharmaceutical company produces a review in its own field. y2="14"></line></svg></button></div></div></div></a></figure></div><p>The document, one of many reviewed and accepted at this medical/scientific meeting, reminds us that comorbidities are common in PKU and may be connected to two realities that move together: on one side, Phe levels that are too high; on the other, the restriction of natural protein required to control them. It mentions issues such as anemia, overweight, obesity, osteoporosis, renal insufficiency, kidney stones, some digestive disorders, cardiovascular disease, diabetes, mood disorders, and cognitive or executive-function difficulties. This list should not be read as an individual sentence. These are not certainties for every person. But they are signals that force us to look at PKU as a lifelong condition, along with other conditions affecting many, not only as a Phe problem to manage meal by meal.</p><p>I do not read this in an abstract way. I read it as someone who lives with PKU, who has a daughter (without PKU), who has worked very hard to build a good life, and who wants to stay here for a long time. I want to be able to play and talk with my daughter and run with my grandchildren. I would like to age in good health, keep my mind, my independence, my energy, and my ability to be present. But when you live with a metabolic disease from birth, you need to think beyond the next blood test. And right now, <a href="https://open.substack.com/pub/pcublogue/p/sephience-5-undeniable-reasons-why?r=47am9s&amp;utm_campaign=post-expanded-share&amp;utm_medium=web">no one really knows what will happen to PKU patients with elevated Phe levels when they reach 80 years old</a> or older.</p><p>This is where comorbidities become very concrete. Obesity, insulin resistance, inflammation, metabolic problems, age-related muscle loss: these are not topics separate from PKU. They are part of the same body. A person with PKU can also age, gain weight, lose muscle, sleep less well, become more metabolically vulnerable, or see their cognitive health become more fragile. PKU does not explain everything, and we should not attribute every health problem to it. But it would be too simple to say that a whole life of dietary restriction, sometimes elevated Phe, and dependence on specialized products has no effect on overall health.</p><p>Sarcopenia, for example, is not at the center of the document, but it is at the center of my concerns. As we age, losing muscle is not about appearance. It is about losing reserve. It means having more difficulty walking, recovering, staying independent, getting through an illness or a harder period. In the general population, there is more and more discussion about the importance of protein, resistance training, and maintaining muscle mass with age. In PKU, that discussion is more complicated, because natural, muscle-building protein is precisely what we restrict.</p><p>Protein substitutes are essential. They have allowed generations of people with PKU to develop, function, work, and live a life. But in real life, it is not the same thing as a freer, more varied, more normal diet with normal nutrition. It is not only a question of nutrition on paper. It is also a question of satiety, pleasure, simplicity, mental fatigue, and one&#8217;s relationship with food. Many people with PKU know the feeling of always having to compensate, always plan, always explain, sometimes always be hungry, or never quite eat like everyone else. <a href="https://www.phenylcetonurie.ca/p/sephience-5-undeniable-reasons-why?utm_campaign=post&amp;utm_medium=web">This constant management has heavy social and cognitive impacts</a>.</p><p>This is the context in which Sephience becomes interesting to me. Not as a miracle solution. Not as a promise that everything disappears. Not as a medication that replaces medical follow-up. But as a possibility to reduce the burden in two places at once: Phe and dietary restriction.</p><p>I have already written about the comparison between Sephience and Kuvan in <a href="https://www.phenylcetonurie.ca/p/sephience-kuvan-et-le-fardeau-du">my previous article on the burden of the low-phenylalanine diet</a>. The essential point remains the same: the <a href="https://www.sciencedirect.com/science/article/pii/S0026049526000223">AMPLIPHY study</a> directly compares sepiapterin, the active ingredient in Sephience, with sapropterin, known as Kuvan. In people who responded to sepiapterin, Sephience reduced Phe more than the maximum approved dose of sapropterin. This matters because Kuvan has helped some people, but it has never transformed life for everyone. For many people with PKU, diet has remained at the center of treatment, with all the burden that comes with it.</p><p>But for me, the most important question comes after the Phe reduction: what can a better reduction in Phe make possible in real life? This is where the <a href="https://researchprofiles.ku.dk/en/publications/effect-of-long-term-sepiapterin-treatment-on-dietary-phenylalanin/">APHENITY extension data</a> become relevant. In that study, some people treated with sepiapterin were able to significantly increase their dietary Phe intake, meaning natural protein, while being monitored. The nuance matters. Sephience does not mean abandoning the diet. The <a href="https://pdf.hres.ca/dpd_pm/00081969.PDF">Canadian Sephience product monograph</a> states that the treatment should be used with a phenylalanine-restricted diet, as recommended by a healthcare professional, and that response must be assessed individually.</p><p>But increasing natural protein in PKU is not a minor comfort detail. It is potentially a life less organized around the disease. Fewer calculations. Fewer specialized products. Fewer separate meals. Less mental load. Maybe better satiety. Maybe a diet that is denser, simpler, and more sustainable. And over time, that margin can become important.</p><p>The conference document does not prove that Sephience will prevent obesity, insulin resistance, osteoporosis, kidney problems, or sarcopenia. That would go too far. Rather, it shows that PKU needs to be considered more globally. It shows that comorbidities exist, that the dietary burden is real, that quality of life is affected, and that restricting natural protein for a lifetime is not simply a medical strategy without consequences. Not only in a logic of a medication that is more effective than Kuvan. Not only in a short-term pricing logic. And not in an overly quick promise of cognitive or physical improvement either. We should instead ask what better metabolic control and greater natural protein tolerance could change in our overall wellbeing over several years.</p><p><strong>PKU often forces us to think in days:</strong></p><ul><li><p>How much Phe today?</p></li><li><p>What blood result this week?</p></li><li><p>How much natural protein in this meal?</p></li></ul><p><strong>But aging with PKU forces us to think in decades:</strong></p><ul><li><p>What cognitive health at 70?</p></li><li><p>What strength at 80?</p></li><li><p>What independence?</p></li><li><p>What metabolic risk?</p></li><li><p>What economic burden for the healthcare system?</p></li><li><p>What quality of life for families?</p></li><li><p>What ability to remain present for the people we love?</p></li></ul><p>That is probably what touches me most in these data. They remind me that I do not only want to survive with PKU. I want to age with as much capability and reserve as possible. Cognitive reserve, muscle reserve, metabolic reserve, emotional reserve. I want to keep my place in my family. I want to be there for my daughter. Always consult a healthcare professional before making changes to your diet or starting a physical activity program.</h6><div><hr></div><p>The original and classic treatment for phenylketonuria has rested for decades on a simple idea: limiting the phenylalanine (Phe) that enters the body.</p><p>In practice, this means a highly restrictive diet very low in natural protein, combined with medical protein substitutes also known as amino acid mixtures. The <a href="https://pmc.ncbi.nlm.nih.gov/articles/PMC5639803/">European PKU guidelines</a> still describe this low-phenylalanine diet, with Phe-free amino acid supplementation, as the foundation of treatment. It has helped prevent the most serious neurological consequences of PKU in children diagnosed early. Even today, it remains an essential part of treatment. But we also need to name a reality that people living with PKU know very well: the low-phenylalanine diet is an imperfect crutch.</p><p>It does not correct the metabolic problem. Instead, it asks the person to build a large part of life around what the body cannot do. Every meal becomes risky and a calculation. Every deviation has to be assessed. Every outing, every trip, every change in routine can become a negotiation with phenylalanine.</p><p>Yes, it is a treatment. But it is also a cognitive, psychological, social, and sometimes financial burden. A study on the financial and time burden of PKU, for example, estimated that families could spend more than 300 hours per year shopping for and preparing special low-Phe foods (<a href="https://pmc.ncbi.nlm.nih.gov/articles/PMC6807265/">Rose et al., 2019</a>). Other work on quality of life in PKU also documents the impact of the disease and its treatment on the daily lives of patients and their families (<a href="https://pmc.ncbi.nlm.nih.gov/articles/PMC4542123/">Bosch et al., 2015</a>).</p><p>This burden is every day and does not last a few weeks. This burden lasts a lifetime.</p><h2>Kuvan: real help, but limited</h2><p>Kuvan, whose active ingredient is sapropterin, was the first way to move partially beyond a strictly dietary logic.</p><p>In some people, sapropterin can help the PAH enzyme work better and reduce blood phenylalanine levels. A Cochrane review concluded that there is evidence of short-term benefit in some people with sapropterin-responsive forms of PKU, including lower Phe levels and a possible increase in natural protein tolerance (<a href="https://pubmed.ncbi.nlm.nih.gov/25812600/">Somaraju et al., 2015</a>). But that detail matters: some people.</p><p>Kuvan has never worked sufficiently for everyone. For many people with more classic or severe PKU, it has not transformed daily life. The diet has remained the centre of and only form of treatment. Kuvan can help some patients, but it does not always meaningfully reduce the concrete weight of PKU.</p><p>This is where Sephience becomes interesting.</p><h2>Sephience: the direct goal remains lowering Phe</h2><p>The <a href="https://www.sciencedirect.com/science/article/pii/S0026049526000223">AMPLIPHY study</a> directly compares Sephience, whose active ingredient is sepiapterin, with sapropterin. In participants who responded to sepiapterin, Sephience lowered phenylalanine more than the highest approved dose of sapropterin. The authors concluded that Sephience was superior to sapropterin in lowering blood Phe, with no new safety signals observed.</p><p>The nuance matters: this does not mean Sephience will work for everyone. Response has to be assessed individually. The U.S. prescribing information states that biochemical response generally cannot be predicted by laboratory testing alone and should be determined through a therapeutic evaluation with Phe monitoring (<a href="https://www.accessdata.fda.gov/drugsatfda_docs/label/2025/219666s000lbl.pdf">FDA, Sephience Prescribing Information</a>).</p><p>But in people who respond, the signal is important.</p><p>Sephience should not be presented as a cognitive treatment in the direct sense. Its primary goal is more specific: lowering blood phenylalanine in people who respond to it. This distinction matters.</p><p>In PKU, a drop in Phe can be measured relatively quickly. The possible effects of better metabolic control on concentration, mental fatigue, mood, or certain executive functions may take longer to appear, when they appear at all. They also depend on many factors: a person&#8217;s metabolic history, age, duration of control, diet, sleep, psychological state, and life context.</p><p>It would therefore be reductive to evaluate a PKU treatment only as if it had to produce an immediately visible improvement in daily functioning within a few weeks. The most direct link to evaluate is first the control of phenylalanine. Then comes the slower, but very important, question of what better sustained control can deliver in real life.</p><p>This nuance is not theoretical.</p><p>In its evaluation of Palynziq, INESSS recognized that PKU causes an accumulation of Phe that is toxic to the brain, that poorly controlled adults may experience neuropsychiatric difficulties, and that the strict diet is difficult to follow. INESSS also noted that Palynziq reduced Phe concentration, but that the 8-week study evaluated did not show improvement in daily functioning or emotional management (<a href="https://www.inesss.qc.ca/en/themes/medicaments/drug-products-undergoing-evaluation-and-evaluated/extract-notice-to-the-minister/palynziq-phenylcetonurie-6128.html">INESSS, Palynziq, 2023</a>).</p><p>This is exactly where careful interpretation becomes essential. A reduction in Phe is a direct metabolic effect. Possible functional or cognitive impacts may be slower, more variable, and harder or impossible to capture in a short observation window.</p><p>In other words: Sephience does not directly &#8220;repair&#8221; cognition. It aims to reduce the body&#8217;s and brain&#8217;s exposure to excessive phenylalanine. If that reduction is maintained over time, it may create better conditions for neurological health, quality of life, and a lighter daily burden.</p><h2>The brain, Phe, and time</h2><p>Imaging data support the idea that metabolic control can have important neurological implications in PKU, even if we must avoid shortcuts. A study published in 1995 observed that some white matter abnormalities seen on magnetic resonance imaging were at least partially reversible when blood phenylalanine concentration was lowered (<a href="https://pubmed.ncbi.nlm.nih.gov/7636650/">Cleary et al., 1995</a>). An important review by Anderson and Leuzzi reached a similar conclusion: white matter pathology in PKU is associated with metabolic control and may be reversible with strict adherence to a low-Phe diet (<a href="https://pubmed.ncbi.nlm.nih.gov/20123467/">Anderson and Leuzzi, 2010</a>). More recently, a study in early-treated adults showed that white matter alterations were related to performance in certain attention and executive function tasks (<a href="https://academic.oup.com/braincomms/article/5/3/fcad155/7161924">Muri et al., 2023</a>).</p><p>These studies do not mean that a medication directly rebuilds white matter. That would be too simple a shortcut. They suggest, rather, that the brain remains affected by the quality of metabolic control, and that Phe should not be treated as just a lab number. In PKU, if a treatment helps keep Phe lower, more often, and for longer, with less effort that is difficult to sustain, it may create better biological conditions. Not a magical promise. Not a guarantee. But a more favourable foundation.</p><h2>Diet normalization is not a minor detail</h2><p>One of the most important aspects of Sephience may not be only the reduction in Phe itself, but what that reduction may allow in daily life. In the language of the APHENITY Extension study, the measured outcome is dietary normalization: an increase in dietary Phe tolerance under metabolic monitoring. But from a patient perspective, the broader goal is dietary normalization. Not necessarily the immediate removal of all restrictions, but a meaningful shift toward a normal, less synthetic diet, less medicalized and less life organization around PKU.</p><p>In that extension study, participants who reached a mean Phe level below 360 &#181;mol/L after two weeks were able to enter a dietary Phe tolerance assessment phase. Among the 102 participants assessed, mean dietary Phe intake increased from 27.6 mg/kg/day at baseline to 62.5 mg/kg/day at week 26, while blood Phe continued to be monitored and remained within current treatment goals</p><p>For a person who has lived with PKU since childhood, eating more normally is not just a secondary comfort. It can mean more natural protein, less dependence on medical products, fewer calculations, less fear around meals, less social isolation, and less decision fatigue. The idea of &#8220;normalization&#8221; may sound ordinary from the outside. It is not ordinary when food has always been an area of acute surveillance because of the threat of Phe.</p><p>Of course, Sephience does not mean the diet disappears. Health Canada&#8217;s approved indication states that Sephience is indicated for patients with sepiapterin-responsive PKU <strong>in conjunction with a phenylalanine-restricted diet</strong>. The Canadian product monograph also specifies that patients treated with Sephience should remain on a Phe-restricted diet as directed by their healthcare provider, based on blood Phe assessment. People will still need to measure, adjust, monitor, and work with a metabolic team. But that does not reduce the importance of the possible change. A treatment can matter even if it does not remove every constraint. It should first be evaluated for what it does directly: lowering phenylalanine in people who respond. Then comes the slower, but essential, question of what better-controlled Phe can allow over time.</p><p>Lower and more stable Phe can support better metabolic stability. Better stability can reduce part of the mental burden. Greater food tolerance can make the diet less invasive. Personally, I face various daily obstacles that could easily be avoided if I didn&#8217;t constantly have to keep my low-protein diet at the forefront of my mind.</p><p>I recently came across <a href="https://globalnews.ca/news/11707867/bc-man-cured-rare-disease-gene-editing-technology/">an article</a> that reminded me that beyond PKU, there are many rare diseases that have an impact on patients&#8217; lives (if not more) than PKU does. The article discussed the journey of Ty Sperle, a young man living with a condition called Chronic Granulomatous Disease.</p><p>But a small technological miracle has come to change his daily life: prime editing.</p><h3><strong>Chronic Granulomatous Disease and Prime Editing</strong></h3><p>Ty&#8217;s disease imposed a mental burden similar to what we experience with PKU.</p><p>In a healthy body, our white blood cells capture and destroy infections, bacteria, or fungi. But in a patient with Chronic Granulomatous Disease, the white blood cells lack the &#8220;ammunition&#8221; to destroy intruders; consequently, the body can only attempt to isolate them (forming what is known as a granuloma). The multiplication of these granulomas can then obstruct airways, damage the skin, and even affect internal organs.</p><p>It is a condition that forces one to live in a constant state of alert, as even the most mundane elements can become dangerous:</p><ul><li><p>The omnipresence of small bacteria or mold poses a threat.</p></li><li><p>Medications are a part of daily life.</p></li><li><p>There is a permanent sense of threat and insecurity.</p></li></ul><p>But this year, Ty Sperle became the first person in the world to be cured thanks to prime editing! This method acts as a way to correct &#8220;spelling mistakes&#8221; in DNA, causing symptoms to disappear. The medications are gone; the mental burden has lifted.</p><blockquote><p><em>&#8220;I also love camping and, you know, it was a risk before, because there are so many bacteria in the woods... so now I can do it without that risk. I have been reflecting on this story for several days, and I can only imagine the happiness parents of a child with PKU would feel if they were told that a gene-editing technique had cured their child.</p><p>We must remain very realistic; such technology is not something that will be working for Phenylketonuria anytime soon. A protein-rich breakfast stabilizes blood sugar, reduces insulin spikes and even improves the glycemic response to your next meal. Like all children his age, he loves to play, laugh, and enjoy life. But Nathan lives with a rare disease: phenylketonuria (PKU). This inherited metabolic disorder prevents his body from properly breaking down phenylalanine, an amino acid found in proteins. To avoid a toxic buildup in his blood, he must follow an extremely strict, low-protein diet every day of his life.</em></p><p><em>Nathan can consume a maximum of 330 mg of phenylalanine per day, or about 7 grams of protein. Every food he eats must be weighed, measured, and calculated. The slightest deviation can have serious consequences for his neurological development, leading to irreversible cognitive disorders.</em></p><p><em>On paper, it may seem simple: all you have to do is count. In reality, it&#8217;s a daily struggle.</em></p><p><em>Let&#8217;s take an example: a typical breakfast for a 9-year-old child, two slices of toast with Nutella and a banana.</em></p><ul><li><p><em>The bread must contain less than 1 g of protein per slice (45 mg of phenylalanine).</em></p></li><li><p><em>The banana adds 45 mg.</em></p></li><li><p><em>A tablespoon of Nutella adds 45 mg.</em></p></li></ul><p><em>Result: 135 mg of phenylalanine for breakfast. That leaves him with only 195 mg for the rest of the day. That includes three meals and snacks. The margin is tiny.</em></p><p><em>Preparing meals, measuring quantities, planning menus, administering the special medical formula... all this takes up nearly 4 hours a day, or 28 hours a week, the equivalent of a part-time job.</em></p><p><em>There are many frustrations. Some days, Nathan is hungrier than usual. But once he reaches his limit, he only has a few very low-protein foods left, which are often tasteless. Temptations are everywhere: at the grocery store, at school, at friends&#8217; parties, on Halloween, or during class celebrations. These are all moments that remind Nathan that he can&#8217;t eat like everyone else.</em></p><p><em>Beyond the diet, there is the emotional weight. Nathan often asks himself, &#8220;Why me?&#8221; He is tired of explaining his condition to justify his &#8220;different&#8221; meals. His need to be accepted, understood, and feel normal is constant.</em></p><p><em>Going on a trip with a child with PKU is a planning challenge. It&#8217;s impossible to rely on the food available at the destination: everything must be prepared in advance, weighed, and transported. Vacation time turns into management and calculation time. Spontaneity does not exist.</em></p><p><em>Fortunately, science is advancing.</em></p><p><em>A new drug, Sephience, could transform the lives of people with PKU. This treatment helps reduce blood phenylalanine levels and increases daily protein intake.</em></p><p><em>For Nathan, this would mean being able to try more foods, go to restaurants, travel more freely, and above all, live like a normal child. This drug represents not only medical hope, but also immense relief for families who spend hours preparing and calculating meals.</em></p><p><em>When Nathan learned about Sephience, his eyes lit up. For him, it is the promise of a simpler, freer life.</em></p><p><em>Sephience must become available to residents of Quebec. Every day counts for children like Nathan, who show exceptional courage and resilience. Thanks to science, they can hope for a more normal life. But for that to happen, these advances must be available to everyone.</em></p><p><em>Let&#8217;s give Nathan and all children with phenylketonuria the hope of a life without calculations, without fear, and full of freedom.</em></p><div><hr></div><h3>Let&#8217;s make our voices heard loud and clear</h3><p>Even when treated with a low-protein diet, phenylketonuria remains a daily challenge: it affects every meal, every decision, every moment of freedom.
